Psychiatric Rehabilitation Program (PRP)

Psychiatric Rehabilitation Services (PRP) is designed to promote healthy development of one’s behavior, social skills, self-management skills, and family relationships. The program is strengths based and therefore strives for the empowerment of the consumer, which leads to them attaining identified personal goals and objectives.

 

PRP services are provided in the setting most conducive to enhancing each ones own healthy participation in the community. This may include meeting in ones home, foster care home, job site, educational setting or other community sites. The intensity of services will vary depending upon the consumer’s individual needs, environment, or diagnosis.

 

With the input of the consumer and family, H.O.P.E Behavioral Health PRP establishes specific goals relating to the areas of personal care, behavioral management, social skills, and other skills that will lead the consumer and his/her family to a more positive future independent of or with less intensive mental health services support. Involvement in the Psychiatric Rehabilitation Program is voluntary and family participation is vital in nearly all cases.

HOW DOES PRP PROCESS WORK?

A licensed therapist MUST refer all clients. Upon receipt of the referral our rehab specialist will conduct a behavioral assessment. The treatment plan will be designed based on the consumer’s diagnosis and assessment information. A rehab counselor is the assigned to the client to begin services.

 

LENGTH OF SERVICES

The consumer will receive 6 visits a month for adults and 4 visits per month for children/ adolescents from one of our experienced Rehabilitation Coordinators. Consumers are reassessed quarterly to ensure that treatment plans remain appropriate to address the 

current service needs.
